The following forms will assist you with requests for accessible text and alternate formats, accessible media (captioning, transcription, audio description), web/software accessibility evaluations, online course evaluations, DS/DEI/Learning Services assistive technology (AT) referrals, AT training, and with reporting physical or information and communications technology (ICT) barriers impacting individuals with disabilities.

PLEASE NOTE: To streamline assistive technology and digital accessibility reporting, DEI is now using the Guardian database to manage all service requests. Unless the link states “For Public Use Only“, a login is required for all form request submissions.

Request AT & ICT Accessibility Support Services

Any Mason faculty or staff member (e.g., instructional designers, web content authors/developers, etc.) involved in the development, creation, and/or publishing of audio or video content that will be used for instructional purposes and/or hosted on front-facing websites may have that content captioned, transcribed, and/or audio-described free of charge.

Please Note: The primary target audience for these resources must be members of the Mason community. ATI staff can assist with resources not targeted toward the Mason community but the requesting unit will be billed for services.

The ATI’s Information and Communications Technology (ICT) Accessibility Standards web page include guidelines to ensure that all information and communications technology resources and services provided by Mason are accessible to people with disabilities. This includes, websites, online learning systems, software and hardware applications, telecommunications, desktop and portable computers.

ATI supports faculty and staff with creating and providing accessible documents for instructional materials and front-facing web resources. Partnering with Disability Services, the Stearns Center, University Libraries, and Creative Services, digital course documents may be sent to ATI staff for an accessibility review and remediation. The ATI provides support for converting textbooks into accessible formats for students, staff, and faculty with print disabilities. To request support, please do the following:

Please Note: Requests are reviewed on a case-by-case basis. Priority is given to textbook requests that are both (1) required and (2) related to a disability accommodation. All faculty and staff requests should be submitted using the document remediation request form. 

Exam and Quiz Requests for Students (FOR DISABILITY SERVICES STAFF USE ONLY)

The ATI offers informal AT assessments for anyone who wishes to visit our office to see what AT is available on campus and to learn about our resources. A referral from Disability Services (DS) opens a new window is needed for any student requesting technology accommodations, such as alternative text, testing accommodations, or other AT to use in the classroom. Faculty and Staff would need a referral from the ADA Coordinator in the Office for Diversity, Equity, and Inclusion (DEI) opens a new window for workplace accommodations.
